Clinical Case Management
We want to support members of our LGBTQIA+ family by offering clinical case management. This service will be introduced in 2025 to help clients who need “check-ins” and those currently waiting on the counseling waitlist.
WHO CAN REQUEST Clinical Case Management?
Any LGBTQIA+ person who physically resides in the state of Texas.
WHAT IS Clinical Case Management?
Clinical Case Management consists of a full therapy session with a clinician on our team. The session can last from 30 - 60 minutes to talk about your experiences during this time. It can be an opportunity to “vent” and talk about your new reality and its challenges. It can also be an opportunity to learn some concrete coping strategies you can immediately implement to deal with your situation. Resource sharing may also be provided, depending on clients request and need.
For clients currently on the counseling waitlist, Clinical Case Management is an opportunity to get support while still waiting for counseling at Out Youth. It is also for clients who may need one-time support. Clients who participate in Clinical Case Management can also access this service more than once.
WHAT Clinical Case Management IS NOT.
This check-in service is NOT to be used for ongoing counseling services. CCM is a one-time session, not weekly. However, you can request CCM multiple times, it just is not guaranteed to be weekly appointments. If you need ongoing support, we will refer you to services at Out Youth or to one of our community partners.
The check-in is also NOT designed for crisis intervention. If an emergency is in progress, please call 911. Please also refer to our mental health crisis resources.
LIMITS OF CONFIDENTIALITY
The following are the limits of confidentiality. These limits apply to check-ins as you will be speaking with a counselor:
Imminent harm in any direction. If you say you are going to hurt yourself or someone else.
Abuse or neglect (current or historic) of a minor, elderly person, or someone who is living with a disability.
HOW DO I REQUEST CCM?
Please complete and submit the request form below. We need to have the following consent on file before we schedule and confirm your request. We recommend completing the consent that applies to you BEFORE submitting your request.
Parent/Guardian Teletherapy Consent (if the client is under the age of 18)
